NCP for the Mother:

Episiotomy:

Pain due to perineal suture

Goal: The client will verbalize a decrease in pain within tolerable limits

Interventions: - Give anelgisics

- Provide bed rest

- Avoid full bladder

- Use ointments

- Antibiotic cream to promote healing

- Frequent changing of perineal pads

- Immediately after delivery apply cold application

- After 24 hrs. Use sits bath for healing episiotomy

- Keep area dry

Breast engorgement {develop after 2-3 days of delivery (pain& enlargement)}

Alteration in comfort R/T breastfeeding

Goal: client will experience a reduce in breast engorgement

Interventions: - Use warm compresses

- Breast feed frequently

- Wear supportive bra

- Massage & manual expression of milk

Cracked Nipple:

Pain related to cracked nipples

Goal: The client will verbalize a decrease in pain within tolerable limits

Interventions: - Applying nipple cream

- Keep nipple dry

- Avoid cleaning with soap

- Always wear a clean bra

Urinary retention:

High risk for altered elimination due to loss of bladder following child birth

Goal: Client will not experience a difficulty in urinating

Interventions: - Perineal care & applying of warm water

- Listen to the sound of water

- Hot water bag to pubic area

- If all managements failed we do catheterization

Sleep disturbance

High risk for sleep pattern disturbance due to exhaustion from child birth & baby care

Interventions: - Provide calm environment

- Give sedation

- Switch off lights

- Drink hot milk

- Encourage bath before sleeping

Edema of Extremities:

Altered perfusion or pain due to poor venous return

Interventions: - Monitor intake & output

- Daily weight

- Advice moderate sodium intake

- Encourage elevating leg

- Avoid sitting for too long

- Massage

Infection:

High risk for uterine infection due to the presence of lochia

Goal: pt will be free of infection during hospitalization

Interventions: - Check V/S 4 hourly

- Encourage sleep & bed rest

- Perineal hygiene & care

- Wash hand before & after perineal care

Altered nutrition:

High risk for altered nutrition less than body requirements due to lack of knowledge about postnatal needs

Interventions: - Eat a wide variety of foods with high nutrients values

- Avoid excessive drinking of milk 1 L is appropriate

- A breast feeding mother should take extra 500 cal after delivery

- Encourage to eat food that are rich in vitamin C & proteins

Child loss:

Grieving due to loss of fetus

Interventions: - Provide emotional support

- Give spiritual support

- Encourage family to sit with her

- Let her verbalize feelings
